For those who seek Vietnam business visa, you are not required to complete any form. Instead, you have to email a passport copy and tentative travel date to support@vietnamvisa-easy.com to request the visa. Important notes: 

Vietnam tourist visa for US citizens is valid from 1 month to 1 year, allows single or multiple entries into Vietnam. The US travelers can come and leave the country anytime during period of visa validity that starts from the expected entry date shown in application.
