For the entry and exit form
For the entry and exit form - it says to fill 2 forms, does that I would need a total of 4 passport photos? or just one for each form for a total of 2?

Dear Tammy,
Please be kindly noted that you should do following instruction as below:
1, Print out the visa approval letter and "Entry and Exit form" (one copy is required for each person) 2. Fill all the information in the Entry and Exit form and then glue your photo on it (4x6 photo is not required). Only one copy is required to show at the airport (you do not need to come to Vietnam visa issuing office or consulate anymore)
3. Bring the visa approval letter and the Entry and Exit form, together with your passport. Present them to Airport Customers Officer upon arrival airport and pay stamping fee in USD in cash (25USD/single entry, 50USD/multiple entry) to get your visa stamped.
